The Member Travel Reimbursement Form is an official record that documents a member's attendance at official meetings with appropriate support. Note that a "member" is a person who is officially designated by NYAHSA/FLTC/NYAHSA Services, Inc./ADHCC to represent the association or the membership during official meetings.
